You can of course also make an appointment by telephone, by email or via our online appointment calendar at doctolib.

BEFORE THE EXAMINATION APPOINTMENT
Data protection, questionnaires, forms
To simplify and speed up the process, we provide you with information and questionnaires to download here.
The data protection declaration is required for every examination. You are welcome to download, print out and complete it at home.
Please also select a questionnaire for computed tomography (CT), magnetic resonance imaging (MRI) or bone densitometry (DEXA) - depending on which examination you are coming to us for.
No additional questionnaire needs to be completed for all other examinations.
Please bring the completed questionnaires with you to your appointment.
Laboratory values (kidneys and thyroid) are required for CT examinations in which contrast medium is injected intravenously. You can have these determined by your family doctor by having a blood sample taken.
We will let you know when you make your appointment whether you need to bring these values with you and whether any medication needs to be discontinued before the appointment.
In the case of spinal pain therapy (facet blockade and periradicular therapy), blood-thinning medication may have to be discontinued after consultation with the attending doctor. We will inform you of this when you make your appointment.
Thank you for your support.
